Where are you located?

I am based out of Mo’ili’ili Fitness (MOFIT) which is located on King St. directly across from Longs. MOFIT is a strength and conditioning gym featuring indoor/outdoor fitness areas, extensive equipment, and a private treatment area. Free street parking is available on Hausten St. or Coolidge St. or metered parking is available on King St.

How do I schedule an appointment?

You can contact me directly through the website (top right corner) or email at mariesakamotodpt@gmail.com to set up a free phone consultation. From there we will discuss your specific needs and schedule the initial evaluation.

Do you accept health insurance? HSA/FSA?

I am an out-of-network/non-participating provider meaning I do not contract with health insurance companies. If you have out-of-network benefits, I can provide a “superbill” upon request, which is a specific itemized receipt you can use to file a claim. Every insurance and plan varies in how much you may be reimbursed, so please verify your plan details if you want to file a claim. Please note, Medicare beneficiaries cannot file a claim as I am not a Medicare provider. Payment is due in full at each appointment. Payments may be made via HSA/FSA card, credit card, cash or check.

Why are you an out-of-network provider?

I decided not to contract with insurance companies for several reasons. Insurance companies often limit what types of treatments and diagnoses are covered, as well as duration of care, which can be particularly frustrating for high-functioning patients and athletes. As an out-of-network concierge physical therapist, I am able to offer the highest quality one-on-one care specific to your needs, goals, and schedule.

Do I need a doctor’s referral?

You do not need a doctor’s referral as Hawai’i is an unrestricted direct access state for physical therapy. However, certain insurance companies do require a referral and/or preauthorization for services. Please verify requirements with your insurance company if you plan to seek reimbursement.

What is your cancellation policy?

Your appointment time is reserved just for you. Please provide 24 hours notice for cancellations or changes. A $60 cancellation fee will be charged for late notices or missed appointments unless extenuating circumstances apply.
